DEIA Feedback Survey

This survey is to aide our DEIA Committee in ensuring we are accountable to our actors, creative team, and instructors and to execute the season of productions and programs in a way that honors our values for diversity, equity, inclusion, access, and anti-racism.  This survey will be reviewed by the DEIA co-Chairs and HR Manager.  Your responses will remain anonymous unless you choose to identify yourself. Responses will be shared with the appropriate parties if the content legally requires action from our HR Department.

A paper version of this survey is available in the Green Rooms at The Francis J. Gaudette Theatre and the Everett Performing Arts Center.  Paper surveys should be sealed in a provided envelope and dropped into the Green Room lock box.  You may also give your feedback by leaving a voice message at 425-392-1942 x309.  To block the phone system caller ID, dial *67 first.  Due to the fact that voices can be recognized, we cannot ensure full anonymity for voice messages.  However, your message will be received via email transcript.  And the voice message will only be listened to by the DEIA Committee Co-Chairs and HR Director, and only if the system transcript is poor.

Please note that this feedback form is not intended to serve as a Human Resources grievance form, but will serve to gather information for assessment of our policies and practices. If you have specific incidents you wish to report to HR, please contact Nikki Singh at nsingh@villagetheatre.org.

Village Theatre wants to make sure all people feel seen and heard and are welcome in our work environment. This weekly survey is intended to aide us in our efforts for accountability, transparency, and good race & social justice practice.
